New for 2025: Chronic Condition Special Needs Plans (C-SNPs)
If you have diabetes, chronic heart failure or a cardiovascular disorder, ask about our NEW C-SNPs.
These plans offer a monthly allowance for healthy food^ and health & wellness items and may include lower copays than some of our other plans for specialist visits and certain prescription drugs.
Is a Peoples Health C-SNP right for you?
There is a special enrollment period for enrolling into a C-SNP.
Peoples Health C-SNPs are for individuals who have diabetes, heart failure or a cardiovascular disorder. If you have one of these conditions, you can use a one-time special enrollment period to enroll in one of these plans at any point during the calendar year.

Regularly see a specialist in our network to manage
one of these conditions
